EHR · Inbox Autopilot · Endocrinology & Primary Care

Most EHRs were built by engineers for doctors. Thyra was built by a doctor who got tired of burning out.

Thyra is a full EHR for endocrinology and primary care. The EHR you can try before you switch. The Smart Inbox demo is live on the homepage with no signup required. Founders can ask JJ a question directly or book a call.

What Thyra is

Thyra is a full EHR for endocrinology and primary care, built by a board-certified endocrinologist who left clinical practice at The Everett Clinic and Optum to build the workflow tool he wished he had. Thyra can run alongside your current EHR (Epic, Athena, eClinicalWorks, Cerner, Allscripts, NextGen, DrChrono, Practice Fusion) as an overlay on day one, then become your primary system on your timeline. One clinical brain across two work surfaces: the chart you document in and the inbox where the rest of the work lives.

Most "AI for clinicians" today is a scribe stitched to point tools that don't talk to each other. Thyra is a single clinical brain: the scribe, the inbox, the CGM viewer, the orders, and the protocols all share the same memory of the patient and the same clinical rules you wrote. That is the difference between a tool that drafts a note and a system that closes a workflow.

Smart Inbox: from message stack to triaged action bundles

The Smart Inbox is Thyra's triage layer. It reads incoming patient messages, lab results, refill requests, prior authorization questions, and faxes, then drafts responses against the clinical protocols you write. Every draft is reviewed by you before it sends. Nothing leaves the chart on autopilot. The inbox is where the unpaid second job of medicine lives, and it is where Thyra spends most of its compute.

Longitudinal AI Scribe: writes what you know, not just what you say

Suki and DAX transcribe what you say in the room. Thyra writes what you know. The Longitudinal AI Scribe pulls prior labs, medication history, visit trends, CGM data, and unresolved issues into a note that reflects the full clinical picture before you finish the visit. Every statement is traceable back to a source in the chart, so you can audit anything in one click.

CGM interpretation, integrated through Tidepool

Continuous glucose monitor data from Dexcom, Libre, and Medtronic flows directly into the chart through Tidepool's umbrella connection. No separate login. No PDF uploads. Time in range, time below range, glucose management indicator, and ambulatory glucose profile show up next to the visit note, ready for interpretation. Supports CPT 95251 billing for personal CGM interpretation, with the documentation Thyra drafts for you.

Telehealth, fax, and e-prescribing built in

Telehealth is a core Thyra feature, not an add-on. Video visits run in the same window as the note. Inbound and outbound fax sit inside the chart, not in a separate inbox. Full e-prescribing, including controlled substances (EPCS), is included with no extra module.

Smart Search and Ask Thyra

Smart Search is Thyra's chart-grounded answer layer. Ask a clinical question about a patient ("last A1C trajectory", "all metformin dose changes in the past year", "any prior thyroid biopsy notes") and get an answer with citations back to the source documents. Built for the seconds before a visit, not the minutes between charts.

Overlay first, full EHR on your timeline

Thyra's overlay-first strategy is a three-phase glide path. Phase one: Thyra runs alongside your existing EHR and takes over the inbox, scribe, CGM, and televisit workflows. Phase two: Thyra becomes the documentation surface, with bidirectional sync to the legacy chart. Phase three: when you are ready, Thyra becomes the primary EHR with full ordering, billing, and scheduling. You do not have to commit to a rip-and-replace migration to get value in week one.

Built by an endocrinologist

Thyra was founded by Jean Jacques Nya Ngatchou, MD, a board-certified endocrinologist with a fellowship from the University of Washington and prior clinical practice at The Everett Clinic and Optum. He left clinical practice to build the EHR workflow tool he wished he had during 12-hour pajama-time nights. Every product decision is made by someone who has carried the inbox.

Backing and compliance

Thyra is backed by INSEAD AI Venture Lab and Google Cloud for Startups. The platform is hosted on Google Cloud. SOC 2 Type II audit is in progress, with completion expected Q2 2026. ONC certification is targeted for Spring 2026. A Business Associate Agreement (BAA) is available before any protected health information changes hands.

Pilot access

The founding cohort is open for endocrinologists and primary care physicians. JJ personally reviews every fit. Decisions in five business days. Pilots include the full Smart Inbox, Longitudinal AI Scribe, CGM interpretation, telehealth, and e-prescribing. Pricing has three tiers: Pilot, Solo, and Pro. End-to-end revenue cycle management is available as an add-on for qualified practices.

Why doctors are switching to Thyra: an AI scribe plus three other tools isn't a clinical brain. Thyra is.

Most "AI for clinicians" today is a scribe stitched to point tools that don't talk to each other. Thyra is a single clinical brain: the scribe, the inbox, the CGM, the orders, and the protocols all share the same memory of the patient and the same clinical rules you wrote.

What clinicians need vs. scribe-plus-point-tools vs. Thyra

Buying a scribe is buying one room. Thyra is the whole practice.

Frequently asked questions

What is Thyra and who is it designed for?

Thyra is a full EHR for endocrinology and primary care. The EHR you can try before you switch. It is designed for endocrinologists, primary care physicians, and practice managers who manage diabetes, thyroid conditions, and metabolic disorders. Thyra can run alongside your existing EHR as an overlay, with no rip-and-replace and no multi-month implementation, then become your primary system when outcomes prove it.

Is Thyra just an AI scribe or a full workflow system?

Thyra is not just an AI scribe. A scribe produces a faster note. Thyra is a full EHR for endocrinology and primary care that manages everything the note generates: inbox triage by clinical urgency, follow-up automation, CGM data integration, and longitudinal clinical documentation across visits. The note is just the beginning. Thyra manages the downstream work.

Can Thyra run on top of my current EHR?

Yes. Thyra can run as an overlay alongside Epic, Athena, eClinicalWorks, and other major EHRs using standard healthcare APIs. It layers on top of your existing EHR without replacing it. Your current system remains the system of record until you decide to switch.

What is an EHR overlay?

An EHR overlay is an application layer that connects to your existing EHR through standardized healthcare APIs, adding workflow intelligence without replacing the underlying system. It reads and writes data through the EHR's native infrastructure. No data migration, no duplicate records, no disruption to scheduling, billing, or regulatory reporting.

How does Thyra reduce after-hours work for clinicians?

Thyra reduces after-hours work, often called pajama time, by turning documentation into downstream action automatically. This includes inbox triage by clinical urgency so physicians address critical items first, pre-drafted responses grounded in patient data, follow-up tasks generated from visit notes, and CGM data embedded in the workflow instead of trapped in a separate portal.

How does Thyra handle CGM data from Dexcom or Libre safely?

Thyra ingests CGM data from Dexcom Clarity, Abbott LibreView, and Medtronic via Tidepool, embedding glucose trends and ambulatory glucose profiles directly into the clinical workflow. The data appears alongside patient messages in the inbox, no portal switching required. Thyra also auto-generates CPT 95251 billing documentation for CGM interpretation.

How do AI EHRs prevent hallucinations in clinical workflows?

Thyra prevents AI hallucinations through deterministic rules, structured data constraints, and full auditability of every AI-assisted action. The AI drafts from verified patient data. The physician reviews and approves. Nothing is sent without human oversight. Every AI-generated recommendation includes source links back to the patient chart.

Is Thyra ONC certified?

Thyra is currently completing ONC certification with 96% of Cypress C1 measures passed. Compliance-first design is foundational to the architecture. ONC certification is targeted for completion as part of the product roadmap.

AI scribe vs EHR overlay, what is the difference?

An AI scribe produces a faster clinical note by transcribing and summarizing the patient encounter. An EHR overlay like Thyra manages the entire workflow that the note generates: inbox triage, follow-up automation, CGM integration, and longitudinal clinical search across visits. A scribe ends at documentation. An overlay begins where documentation ends.
